The following student self evaluation transfer credit forms are required for students who would like to apply previously earned credits. Please select the form for the program you are interested in.

NOTE: Certified Law Enforcement Personnel enrolling in the Criminal Justice Programs are exempt from paying the Application Fee and Credit Evaluation Fee.
